HOW DO I...
REGISTER?

Patients wishing to register with the practice should come into the health centre and ask to see a member of staff from the computer room. A registration form and medical questionnaire will be issued for each family member. On completion, these forms should be given back to the computer room to be checked and to ensure that continuity of medical services is maintained.

MAKE AN APPOINTMENT?

New Appointment Arrangements
Following concerns about the length of time patients have had to wait for doctors' appointments, a new appointments system has been introduced.

This means:
We now ask our patients to phone on the day they require to be seen by a GP. The practice will always try to give you an appointment with the doctor of your choice though this may not always be possible. All appointments are for a minimum of 10 minutes. Nurses and health visitors continue to operate as previous.
To make a same day appointment, please call early.

HOW DO I...
MAKE COMMENTS OR SUGGESTIONS ?

We are happy to accept and consider comments and suggestions from our patients. Please present your views in writing at reception or speak to our practice manager.

HOW DO I...
MAKE A COMPLAINT ?

We always try to provide the best service as possible, but there may be times when you feel this has not happened. The following information explains our in-house complaints procedure, drawn up to respond to patient grievances. Our practice procedure is not able to deal with questions of legal liability or compensation. We hope you will use it to allow us to look into and, if necessary, correct any problems that you have identified, or mistakes that have been made. If you use this procedure it will not affect your right to complain to the Health Board.

If you wish to make a complaint, please telephone or write to one of our practice managers. Full details will be taken and a decision made on how best to undertake the investigation.
